Do not alter or change the form once it has been filled out and signed. If possible, take photographs of the accident scene, the damage to the vehicles, and the license plate of the vehicle belonging to the party at fault. <\/p>

The set of a paper traffic accident declaration consists of two identical self-copying forms, so when filling out the declaration, it is not necessary to fill in both forms separately, it is enough for both participants in the traffic accident to fill out the first declaration form – each of them has their own part. If you use a copy of this form, fill out as many pages as there are participants in the traffic accident. <\/p>
